Note:

Many of the countries to which we’re traveling require Parental Authorization Forms for travelers under the age of 18. These forms must be completed and notarized within 30 days of departure, and generally require the signatures of both custodial parents or guardians. However, if only one custodial parent or guardian is able to sign, simply complete this form accordingly.

Parent(s) or Guardian(s) of those travelers who will be under 18 for any portion of our itinerary are asked to please meet at the Starbucks located near Forest and Webb Chapel (3100 Forest Lane, Dallas) anytime between 4:30 - 5:30 p.m. this Saturday, June 21st in order to sign the form.
